Print this checklist before an enrollment meeting, parent-teacher conference, campus tour, or doctor\u2019s appointment. Knowing what to ask \u2014 whether for yourself or a child \u2014 turns a vague worry into a concrete safety plan.
Childcare / Preschool
For a child in daycare, preschool, or an in-home provider.
- Do you have a written food-allergy and anaphylaxis policy, and is staff trained to recognize and treat a reaction?
- Do you stock undesignated (backup) epinephrine, and who on staff is authorized to administer it?
- Where will my child’s prescribed epinephrine auto-injector be kept, and how quickly can staff reach it?
- How are meals, snacks, and treats handled to prevent cross-contact, and do you prohibit food sharing?
- Is the classroom cleaned (wiped tables, hand-washing) before and after eating?
- How are field trips and special events (birthday parties, holidays) made safe for my child?
- How will you handle a reaction if [parent/child name] is not onsite — who calls 911 and tells EMS the allergens?
- Can an individualized emergency/allergy action plan be on file, and is it reviewed annually?
K–12 School
For a student in elementary, middle, or high school.
- Is there a full-time school nurse, and who covers when the nurse is absent?
- Is there a 504 plan, IEP, or Individualized Healthcare Plan, and how is the allergy written into it?
- Does the school stock undesignated epinephrine under your state’s stock-epinephrine law, and who is trained to use it?
- Where is my child’s prescribed epinephrine stored — with the nurse, in the classroom, or self-carried — and who has access?
- Is self-carry/self-administration of epinephrine and an inhaler allowed and documented?
- What allergen-aware practices are used in the classroom, cafeteria, and on the playground (designated tables, hand wipes, no food in class)?
- Are teachers, substitutes, bus drivers, coaches, and after-school staff trained on the plan and emergency response?
- How are field trips, class parties, science labs, and shared supplies made safe?
- Is there a "no-bullying around food allergies" policy, since food-allergy bullying is a documented risk?
College / University
For a student transitioning to campus life and self-management.
- Does the disability services office (e.g., DRC, ODS) provide reasonable accommodations for food allergies and asthma, and what documentation is required?
- Are dining halls able to provide allergen-free options, clear labeling, and prevent cross-contact — and can I meet the dining dietitian?
- Can the student live in a residence hall close to dining and the student health center, with a roommate who understands the allergy?
- What emergency response is on campus — is there an on-call nurse, stock epinephrine, and how fast is EMS?
- Does student health manage and store the student’s epinephrine and inhaler, and can they refill prescriptions?
- What training do resident advisors (RAs) receive on anaphylaxis recognition and epinephrine use?
- Are classroom, lab, club, and Greek/athletic settings (field trips, banquets, tournaments) covered by the accommodation plan?
- Is there an Emergency Action Plan filed with campus safety, and does the student self-carry epinephrine and a medical ID?
- What is the process for study-abroad and travel programs regarding epinephrine, insurance, and food safety?
- What insurance does the student health center take, and what happens for after-hours emergencies?
Doctor / Allergist / Care Team
For yourself or your child — when you are newly diagnosed or revising an action plan.
- What exactly is my (or my child’s) diagnosis — which allergens/triggers, and how severe has the reaction history been?
- Which tests confirm this (skin-prick, specific-IgE, oral challenge, component testing) and do we need to repeat or refine them?
- What are my treatment options — strict avoidance, oral immunotherapy (OIT), biologic therapy, or desensitization — and what are the risks and benefits of each?
- When and how do I use epinephrine, and how many auto-injectors should I carry (and who else should have one)?
- Do I need an Anaphylaxis Action Plan / Asthma Action Plan / Food Allergy Action Plan — and can you provide copies for school, work, and home?
- What are the warning signs that mean I should go to the ER even after epinephrine, and what is the follow-up plan after a reaction?
- For asthma: which is the daily controller, which is the rescue inhaler, how do I use them correctly, and when do I step up or down?
- What is the plan for re-evaluation — when might my child outgrow an allergy, and when should we retest?
- Are there relevant clinical trials, and would this condition benefit from a referral to a specialist (pulmonology, GI for EoE, dietitian)?
- What is the cost of my medications and what lower-cost options exist (generic epinephrine, manufacturer copay cards, patient-assistance programs)?
Tip: bring a copy of your Action Plan (food allergy, anaphylaxis, or asthma) to every meeting and ask the setting or provider to sign that they received and reviewed it.
