An international, multicenter cohort study of 558,823 platelet and plasma transfusions found that in U.S. regions with high alpha-gal syndrome prevalence, blood type O patients receiving group B or AB units had significantly higher rates of allergic transfusion reactions (RR 3.93) — with moderate-to-severe reactions reaching RR 9.14. The study provides provocative epidemiologic evidence that TRAGS may be a unique, previously unrecognized transfusion risk, and recommends risk mitigation in AGS high-prevalence regions.
